[NLPL Task Force (A)] application form for new projects and accounts on Taito

hi martin,

the NLPL wiki links to a CSC page where there used to be a link to the
PDF file for acccount applications, i.e.

but it appears the PDF account application form for university users
form outside finland is no longer linked from that page.  i can find a
very similar-looking form on the sub-page for research institutes and
companies, only it says at the top of the form ‘commercial customers’,
which does not quite sound right, strictly speaking.

is the old non-commercial paper version of the application form still
available somehwere?
